為辦理本組各項業務之目的，需蒐集您的個人資料

（如：C001、C002、C003、C011、C012、C013、C014、C021、C023、C033、C034、C035、C038、C051、C052、C057、C061、C064、C071、C072、C081、C111、C113、C120），

在業務執行期間於本校校務地區進行蒐集、處理、利用個人資料，繳交文件視同同意本校將您的個資使用於該項業務，如有欄位未填寫，可能對您的業務權益有所影響。您得依個人資料保護法第3條行使查閱、更正個資等當事人權利，行使方式請洽本校教務處註冊組(分機 4013-4019）。

個人資料保護法第3條規定：當事人對其個人資料依本法規定行使之下列權利，不得預先拋棄或以特約限制之：

一、查詢或請求閱覽。

二、請求製給複製本。

三、請求補充或更正。

四、請求停止蒐集、處理或利用。

五、請求刪除。

Notice Regarding Personal Data Collection:

For the purpose of processing various services provided by this group, we need to collect your personal data(e.g., C001, C002, C003, C011, C012, C013, C014, C021, C023, C033, C034, C035, C038, C051, C052, C057, C061, C064, C071, C072, C081, C111, C113, C120).This data will be collected, processed, and used within the school's administrative area during the service period. Submitting this document constitutes your consent to the school's use of your personal data for this service. Failure to fill in any fields may affect your rights and interests. You may exercise your rights to access and correct your personal data in accordance with Article 3 of the Personal Data Protection Act. Please contact the Registration Section of the Academic Affairs Office (Ext. 4013-4019) for instructions on how to exercise these rights.

Article 3 of the Personal Data Protection Act stipulates that the following rights of individuals to exercise their personal data in accordance with this Act may not be waived in advance or restricted by special agreement:

1. To inquire or request access.

3. To request supplementation or correction.

4. To request cessation of collection, processing, or use.

5. To request deletion.
